There’s a tiny issue with Call of Duty: Black Ops 4 on PC while choosing the right Refresh Rate for your monitor.
Players have reported that they’re only able to choose the 60Hz and 24Hz options from the in-game graphics settings while playing in Full Screen.
This is faced mostly by people who have multiple monitors connected, so to fix this just disconnect all monitors other than your main one. Disabling them and then restarting the game should give you access to all of the other Refresh Rate options.
