Fallout 4 has a lot of bugs and glitches from harmless to game breaking bugs. One of these glitches results in the removal of the player’s head making him essentially a headless monster.
This glitch happened to reddit user postbroadcast as he encountered a Deathclaw who managed to rip his head off but he still survived that attack, resulting in the game glitching out with a headless player.
You can check out the video made by the user below, which shows him exploring the Wasteland as a headless Hunter and talking with various NPCs, who don’t seem to have any reaction to the headless player.
The user explained that he did this glitch without any hack or mod. The glitch sadly wasn’t saved and once he reloaded his save, the head was back where it belonged.
