Unfortunately Shadow of the Tomb Raider’s Platinum Trophy “The Completionist” is glitched at the time of writing with the latest patch.

Players are reporting some collectibles simply won’t show up while the Map’s 100% exploration trophy would fail to pop up after completion. So if you’re scratching your head wondering what you’re doing wrong, then don’t worry you’re not alone.
Shadow of the Tomb Raider developers Eidos Montreal should be fixing this in a patch which should land any day now.
The trophy’s main challenge is getting the 378 Collectibles and doing the 15 Challenges, with each Challenge on average having 3-5 things to collect. Luckily none of these items are missable and you can always go back to get them.
The best way Shadow of the Tomb Raider trophy hunters have found to get these items is to go for them after beating the main game, as you’ll have complete free-roam to get wherever you want including Cozumel, a location you can’t go back during the main story after the game’s opening.
These items are much easier to find if you play on the Smart and Resourceful or Easy difficulty. This will allow you to use Survival Instinct with R3 to find interactable objects.
You can also upgrade the Seeker skill tree to get Eagle’s Slight, Owl’s Wisdom and Owl’s Vision to make finding items and challenge objectives easier and make Survival Instinct last longer.
